Avanti Study Summary
Criteria included seven or more maintenance medications and a diagnosis of diabetes, heart failure and/or COPD
Fifty patients meeting criteria were enrolled and followed for six months
Intervention began with a comprehensive pharmacist-driven chronic illness management review followed by recommendations to prescribers adopted at a rate of 66%
Avanti delivered fully optimized medication regimens to patients within 10 days of initiation of services
Patients continued to receive ongoing patient-centric medication management to ensure they continued to adhere to their regimens with reduced risk of medication errors
Results included reductions in nursing home admissions (66%), hospital readmissions (41%) and emergency room visits (16%)
